         Psychopathology does not predict hypersensitivity in IBS

    The relationship with clinical symptoms and psychological factors has not been fully established. A research assessed which of these factors predicts the occurrence of visceral hypersensitivity in irritable bowel syndrome. It was found that rectal compliance was significantly reduced in irritable bowel syndrome patients compared with controls, as were thresholds for pain and urge. Levels of anxiety, depression, neuroticism, somatization, and dysfunctional cognitions were significantly increased in irritable bowel syndrome patients vs controls. In contrast, pain coping and quality of life were significantly worse.

    Hypersensitivity to balloon distention occurs in 33% of irritable bowel syndrome patients and is predicted by symptom severity but not by psychological or demographic characteristics.

         Warm water vs oil-assisted colonoscopy

    The researchers randomized 3 groups of patients to colonoscopy. Group 1 included 170 patients who received a standard lubricating method of a water-soluble jelly. Group 2 included 170 patients who received the standard method plus injection into the colon of corn seed oil.

    The researchers found that successful intubation to the cecum was significantly more frequent in the oil group (Group 2), and in the warm water group (Group 3), and less time was needed. The team found no significant differences with regard to time for examination at withdrawal among the 3 groups. Level of pain, and degree of difficulty during colonoscopy were significantly lower in the oil, and in the warm water groups. Neither side effects were observed for patients, nor damage to the instrument. 

    Dr Brocchis' team concluded, "Warm water, and oil-assisted colonoscopy could be simple, safe, and inexpensive methods for easier and less painful examinations."
